Diabetes

Provided for people registered to GP practices within Nottingham West Clinical Commissioning Group.

The Diabetes service is an integrated pathway with hospitals, mainly Queen’s Medical Centre and City Hospital in Nottingham. The service has diabetes consultant links with specialist diabetic nurses that repatriate patients with type 1 and 2 diabetes from hospital to be looked after in the community.  The service has helped in terms of initiation of insulin and insulin analogues in the community and a reduction in outpatient attendance at hospital diabetic clinics.

The service has resulted in:

  • Most diabetes care provided in the community, with  reduced referrals to hospital.
  • Improved integration of GP practice and hospital care of diabetes.
  • Reduced health complications caused by diabetes.
  • More health professional, including doctors and community nurses, skilled in caring for diabetes cases.
